Do you have virtual openings?

Yes, I am happy to see you virtually.

What is the next step?

I recommend making a consultation appointment to see if we are a fit for therapy.  These sessions run a normal 50-minute length and give us a chance to see if we are a match for working together.

There is no charge for this session.

Truthfully, it is best to interview a few therapists to see who is the right match for your situation.  I am always happy to meet so that you can experience first-hand my style of therapy.  Frankly, this gives you the opportunity to meet me, ask questions, and learn more about what I do; this consultation allows you to make an informed decision about your next steps. If I am not a match for therapy, I am happy to recommend other providers who may be a better fit for your situation.

What is the cost of couples therapy?

$200/50 minute session

$300/90 minute session

What is the cost of individual therapy?

$150/50 minute session

Is this therapy covered by insurance?

In some cases, yes.

I do not participate with any insurance providers (I am “out-of-network”). However, my services may still be covered if you wish to use your insurance benefits.

Most patients prefer the privacy of self-pay. Still others want to use their health insurance benefits.  I encourage you to call your insurance company to verify your out-of-network benefits ahead of time. It is important to know that if you choose to utilize your insurance, the insurance company has a right to request records of your sessions.  Complete confidentiality occurs only with self-pay.

If you are unsure of your insurance benefits, I recommend you call your insurance provider prior to your first appointment.

The following questions may be helpful to ask your insurance provider:

Do I have out-of-network outpatient* mental health benefits for family therapy? (*Outpatient means treatment outside a

hospital.)

If yes:

  1. What is my out-of-network deductible for outpatient mental health?

  2. How much of my deductible has been met this year?

  3. Do I need a referral from an in-network provider to see someone out-of-network?

  4. What is the allowed amount for out-of-network outpatient mental health sessions?

  5. Do my insurance benefits cover outpatient mental health services for family therapy (CPT Code 90847)?

  6. Do I have a deductible to meet first or a required copay per session?

  7. What will it cost me per outpatient family therapy session?

  8. How do I submit my therapy bills ("claims") to you for reimbursement?

    1. What is the form I use & where is it located?

  9. Can you send me a copy of my out-of-network outpatient mental health coverage for my records?

  10. May I have a reference number for this phone call?

  11. Any other questions or concerns for your insurance plan to help with your decision making.

When verifying your benefits with your insurance company, it is best to use my specific name: Sarah J. Tomakich LMSW

Self Pay
I accept cash, checks, credit and debit cards as well as HSA and other savings cards as payment. I provide you a confidential portal for payment once we complete the intake process.

If you have any questions or concerns about payment, I encourage you to discuss them with me during our consultation session.

Payment Policy

I ask for payment to be made directly to me. If you wish to submit to your insurance company for reimbursement, I will furnish you with a statement at the end of each month with all of the required information on it.

A Note on Managed Care Plans

There are a variety of reasons that I do not participate in managed care plans. Specifically, insurance companies require that all individuals receiving any type of mental health treatment receive a psychiatric diagnosis. Additionally, once your insurance company obtains your mental health treatment information, I can no longer guarantee its confidentiality. Most importantly, insurance companies often influence the type and duration of the course of therapy, and I strongly believe that you and I together should be the ones that determine the best way to help you meet your objectives, not your insurance company.
